新冠感染者中的无症状感染者的比例问题一直广受关注。本月初，广州市一场疫情防控新闻发布会上公布的数据显示，本轮疫情广州市的无症状感染者占感染人群总数约9成。近日，多位专家、学者也表达了随着奥密克戎致病性减弱，以及疫苗接种的普及，无症状感染者占比较高的观点。而随着感染者越来越多，并向身边的亲朋好友分享与新冠病毒作斗争的经历，许多民众发现似乎“身边都是轻型感染者，无症状感染者的人数很少”之所以会形成这种感觉，因为检测策略和防控措施发生了改变，以前基本都是核酸检测，核酸检测灵敏度高于抗原检测，而现在许多居家隔离的民众开始采用抗原检测，许多无症状感染者通过抗原检测试剂盒无法测出，这就造成有症状的感染者占比较多的“错觉”。
此前公布的新冠肺炎诊疗方案明确指出，新冠无症状感染者为核酸检测呈阳性，但无相关临床表现，如发热、干咳、乏力、咽痛、嗅（味）觉减退、腹泻等可自我感知或可临床识别的症状与体征，且CT影像学无新冠肺炎影像学特征者。而轻型病例则指核酸检测呈阳性，临床表现较为轻微，通常表现为发热、呼吸道感染、轻微乏力等症状，一般不会出现呼吸困难、胸闷等表现，影像学无肺炎表现。
“但现在很多人可能感觉到，我们身边的检测出的感染者很多都是有发热、咳嗽、咽痛等症状的，这似乎与我们此前宣传的还有大量无症状感染者的情况不符，其实两者并不矛盾”，吴浩指出，此前我们采用“核酸检测阳性”作为确诊新冠的金标准，根据国家第九版新冠诊疗方案，一旦核酸试剂的CT值小于40就判定为核酸检测为阳性，大于40则为阴性。现如今我们采用抗原检测法作为辅助检测手段，实践观察发现，一般而言感染者核酸检测的CT值要小于28，在进行抗原检测时，试剂盒的“C”和“T”处才会显示出红色或紫色条带，此时抗原检测才可能筛查出阳性感染者。两种检测方法的不同，造成了许多核酸检测为阳性的无症状感染者，采用抗原检测就会是“健康人群”，“实际上，许多体内病毒载量较低的无症状感染者，抗原检测是查不出来的。现实情况还发现，抗原检测阴性者即使核酸检测呈阳性，载毒量也低，传染力也弱。”
在新冠肺炎第九版防控方案中，首次明确基层医疗机构可对可疑患者增加抗原检测，而随着新冠防控方案的逐步优化，核酸检测范围和频次逐渐缩小，并鼓励有自主抗原检测需求的人员可以按照自主、自愿的原则，随时进行自我抗原检测。因此，许多居家隔离的民众开始采用抗原检测法来判定自己和家人是否感染新冠。
“如果我们采用抗原检测，一则是感觉到自己有被感染的风险，二则是发现自己出现了症状因此才开始检测，在这种情况下，就会是有症状感染者看起来似乎更多，因为许多无症状感染者并没有寻求筛查。”吴浩称。
吴浩指出，现在我们需要更理性地去看待疫情防控措施的调整。经过近三年的科学抗疫，许多人已经养成了保持合适的社交距离、戴口罩、勤洗手等良好的卫生习惯，这些良好的卫生习惯在未来都可以极大地帮助我们避免病毒的传播。
对于防疫政策的调整和逐步优化，吴浩表示，这绝不是单纯的“放开”，更不是“躺平”，而是在考虑经济生产活动和民众正常生活之后的统筹和平衡，“公共卫生政策的制定，一定是全方面考虑后出台的策略。奥密克戎已呈现传播力强，但致病力相对较低的特点，如果我们再用传统的防疫方式去应对，那么所付出的代价将难以承受。未来针对新冠病毒特点我们要不断优化调整防疫政策，走小步，不回头。现在我们要抓紧时间做好准备工作，包括快速推进老年人的疫苗接种工作，加强医疗物资储备，加强基层医务人员诊疗能力培训，重点关注广大农村地区的新冠防治以及儿童的防治工作，针对病毒变异特点优化措施，加大科普及时回应民众关切和疑惑，消除恐慌情绪，防止医疗挤兑，把有限的医疗资源用在‘刀刃’上，减少重症及死亡病例的发生。”
